By making a contribution to the Homeopathic Medicine PAC, you affirm that:

Your contribution is made from your own personal funds and not from the general treasury of a corporation, labor organization, or national bank (for the Contributory Fund).

Federal law requires us to use our best efforts to collect and report the name, mailing address, occupation, and employer of individuals whose contributions exceed $200 in a calendar year. You agree to provide accurate information for these reporting purposes.
